Investigations conducted on fires and explosions at refinery plants under study indicate that there is little or no strategy to deal effectively with the threat of fires and explosions at refinery plants. The incidents of fires and gas/hydro-carbon leaks appear to be unrelated to anyone with knowledge of them. This study plans to locate and identify the underlying causes of fires at refineries. A regression equation is one tool used to define, measure, and analyse data from the Refinery in South Africa (RSA), and it was found that the equation has a significant overall goodness of fit. There are several improvements required for the control of equipment and general communication, while the moral compass of employees needs to be improved. Many constraints were experienced due to limited access to the refinery. The data was collected from thirty-five employees. The findings of this research will serve as a basis to develop successful defensive mechanisms to eliminate, control and minimise risks to non-significant levels.en_US
